Drive to Succeed

To be eligible to apply, applicants (students) must:

• Meet one or more of the following criteria:
  • School reduced/free lunch participation
  • WIC/SNAP household participation
  • Medicaid enrollment
  • Eligible for one of the above, but not enrolled
• Be between 15-1/2 to 20 years of age
• Live in Licking County

AND

Submit the following:

1. Completed Drive to Succeed Application via Microsoft Form (link provided at the top of the page)
2. Drive to Succeed Application - Essay and Affidavit emailed to dts@lickingcohealth.org
3. Proof of driver’s permit emailed to dts@lickingcohealth.org
4. Proof of student eligibility (refer to the criteria listed above) emailed to dts@lickingcohealth.org

If approved, a $10.00 fee is required. LCHD will notify you when payment is due via invoice.
