Drive to Succeed

-To be eligible to apply, applicants (students) must:

• Meet one or more of the following criteria:
  • School reduced/free lunch participation
  • WIC/SNAP household participation
  • Medicaid enrollment
  • Eligible for one of the above, but not enrolled
• Be 16 or 17 years of age at the time of applying and complete the training before they're 18
• Live in Licking County

AND

-Submit the following:

1. Completed application
   • Including signed parent/guardian affidavit
2. Proof of driver’s permit
3. Proof of student eligibility (refer to the criteria listed above)

We will contact you if the applicant has been approved.

• Print and drop off at Licking County Health Department (same address as above)
  • Hours of operation: Monday – Friday, 8:00 AM – 5:00 PM
    • Closed for Federal Holidays
• Emailed to dts@lickingcohealth.org

Once the application is approved a $10 fee is required
