Our goal is to help create a healthier Licking County by partnering with the community to connect individuals with the support they need.
How We Help
Our staff, who are also members of the community, will help understand your needs and work with you to create a plan using available resources. When needed, the navigator can advocate on your behalf to ensure you get the support you deserve.
We will help you identify your needs and work with you to create a plan to address those needs by navigating existing community resources.
Community Navigator Referral Form
The Community Navigator helps individuals and families overcome barriers to their basic needs, empowering them to navigate systems independently. By meeting people where they are, the Navigator provides support that helps prevent reliance on substance use or unhealthy coping mechanisms.
With a deep commitment to fostering connection and growth, the Navigator walks alongside each person, guiding them toward positive change. If you or someone you know would benefit from this type of assistance, please complete the questions in this form to refer them to the Community Navigator program.
