Patient Navigator Program
Medical and Dental Case Management and Referral (Patient Navigator) helps those in need of medical or dental care access the health resources available in Licking County. The Patient Navigator (case manager) makes referral to health care providers and agencies that accept Medicaid or those that offer services for free or on a sliding fee scale. Limited funds are available through the Partnership to assist with lab and materials costs for health care providers that volunteer to see clients referred through this program. Child Car Safety Seats
Car seats are required by law in Ohio for children under 3 years and less than
30 lbs. Car seats are provided for low income families in need of seats through
the Licking County Health Department's Child Safety Seat program. Funds donated
to the Partnership are used to purchase additional seats for distribution to
Licking County families. Parents are trained on
proper installation and use of the seats and the seat is properly secured in the
car by a trained car safety seat technician. Seats
are also provided though the Ohio Buckles Buckeyes program, PRC funding through
Licking County Dept. of Jobs and Family Services, and donations from local
organizations.
